This option facilitates individual contractors and single person companies who are or want to become members of CMDS.

How to Sign Up
For a contractor to become a member of the CMDS group, click on the “sign up” button at the bottom of this page, read the confidentiality agreement, and at the end of the document click on the “I Agree To The Terms” button.
The contractor will be presented with a document which needs to be printed, filled out and mailed to the address on the form. Following the sign up procedure the contractor’s contact information will be listed in one of the occupations within the “Individual Contractor” link.
